Intraindividual night-to-night variability using Peripheral Arterial Tone (PAT) technology - How does the proportion of supine position and REM Sleep vary?

    Introduction For the treatment decision and in research in patients with sleep apnea (OSA), it is important to reduce possible distortions such as the night-to-night variability. In particular, reference to REM sleep or supine position are important. In a study on the prognostic value of MRI of the upper airway with functional flow analyzes (I-SLEEP, funded by VDI, Technologiezentrum GmbH), the OSA is determined with two measurements to evaluate the effects of mandibular protrusion. The aim of this substudy is to determine intraindividual variability in OSA severity.

    Materials and methods The inclusion of 25 probands In the study is planned. The initial and final measurement will be performed on two consecutive nights with PAT technology (WatchPAT ©). Therefor in addition to the position assessment of REM sleep is possible. Variability was defined with a deviation of ≥ 10 %.

    Results 11 patients (40 ± 16 years, 8 female, BMI 25.8kg / m² ± 5) with an average AHI 14.4 ± 9.9. Of these, 55 % had a mild, 36 % medium and 9 % a severe OSA. A relevant variance was found in 78 % of patients for total AHI and REM AHI, 71 % for supine AHI and 100 % for ODI.

    Conclusion/discussion In this preselected patient collective, a striking intraindividual night-to-night variability was shown in various OSA parameters. With the use of tonometry system, different parameters can be quantified with less distorsion by performin a double measurement. This seems to be suitable even with smaller cohorts with and in particular for less severe OSA. Recruitment is continued until presentation and the therapy effects to be measured are updated.
