Kognitive Defizite bei Depressionen und ihre Behandlung

  • Kognitive Defizite bei depressiven Patienten können in allen wichtigen neurokognitiven Domänen auftreten und bei ca. 30 – 50 % der Betroffenen über die akute Phase hinaus persistieren. Sie können Alltag, Berufsleben und Lebensqualität Betroffener beeinträchtigen.

  • Die gezielte Therapie von kognitiven Defiziten mittels kognitiver Remediation und ggf. pharmakologischer Strategien sollte die übliche Behandlung ergänzen.

  • Eine kognitive Remediation besteht aus regelmäßigen restitutiven und kompensatorischen Interventionen, z. B. aus massierter Übung am Computer und ergänzendem Transfertraining.

  • Was die Effekte von Antidepressiva auf kognitive Defizite betrifft, ist die Befundlage inkonsistent, daher gibt es aktuell noch Forschungsbedarf.

  • Es gibt Hinweise darauf, dass Medikamente mit anticholinergen Eigenschaften die Kognition negativ beeinflussen, während die adjuvante Therapie mit kognitionsverbessernden Substanzen bei depressiven Patienten ein vielversprechender Ansatz in Bezug auf die Verbesserung von kognitiven Defiziten sein könnte.
