Kernaussagen
  • Angioödeme stellen Notfallsituationen dar und bedürfen nach der Erstversorgung einer klinischen Überwachung.

  • Nach Korrektur der Vitalfunktionen sollte möglichst früh eine kausale Therapie der individuell vorliegenden Angioödemform eingeleitet werden. Die wesentliche Unterscheidung geht von den Hauptmediatoren Histamin bzw. Bradykinin aus.

  • Das Angioödem mit Urtikaria ist geprägt von Quaddeln und Juckreiz und ist die häufigste Form des Angioödems. Es spricht auf die Gabe von Adrenalin, Antihistaminika und Kortikosteroiden gut an.

  • Das durch ACE-Hemmer induzierte Angioödem ist eine nicht seltene Nebenwirkung, bedarf nur vereinzelt einer iatrogenen Atemwegs sicherung und kann probatorisch mit C1-INH oder Icatibant behandelt werden.

  • Das eher seltene hereditäre Angioödem (HAE) ist bradykininvermittelt und spricht auf die Gabe von C1-INH und des Bradykinin-B2-Rezeptor-Antagonisten Icatibant an.
