Introduction Bilateral vocal fold paralysis (BVFP) is a rare disease mainly of iatrogenic origin that in most cases critically reduce the respiratory patency of the affected patients and in the worst cases may become life-threating. Currently it is mainly diagnosed by medical history and video(strobo)laryngoscopy assessment. Its current golden standard is surgical glottal enlargement or tracheotomy in emergency cases. No standardized diagnostic, treatment, and follow-up protocol exists for this disease, thus the comparison among the results obtained worldwide is difficult, as we showed in our previous publication of 329 retrospective cases within an international registry conducted in 11 hospitals in EU and Ukraine

Methods Currently we are prospectively collecting BVFP cases in 15 hospitals, including quality of life (QoL) questionnaires. The hospitals are trying to perform respiratory and voice quality assessment as well as administer QoL questionnaires pre-treatment and at regular visits within 2-year post-treatment

Results 47 patients have been prospectively enrolled so far. 28 patients completed the 6m; 19 the 12m; and 13 then 24m follow-up. For these patients at least a voice quality and respiratory test, a QoL, a voice, a respiratory, a swallow, a sleep quality questionnaire are available at baseline and one or more follows-up

Conclusions The results of the prospectively recruited patients shall provide a better insight on how the collection of regular objective voice and respiratory assessment and of subjective QoL questionnaire can help the follow-up of BVFP patients and increase their compliance with post-treatment follow-up.
