Die operativen Standardverfahren zur Behandlung des Lungenkarzinoms sind Segmentresektionen,
die Lobektomie einschließlich ihrer Modifikationen mit angio-, bronchoplastischer
Manschettenresektion, die Bilobektomie und die Pneumonektomie. Die Entscheidung zur
Ausdehnung der Resektion unter Mitnahme angrenzender Strukturen (Brustwand, Herzbeutel,
Vorhof) wird der intraoperativ angetroffenen Situation angepasst. Erweiterte Resektionen
von Nachbarorganen (thorakale Aorta, Ösophagus, Wirbelsäule) stellen bei fehlender
Evidenz eines therapeutischen Benefits für den Patienten die Ausnahme dar. Es muss
bei der Operation immer eine systematische interlobäre und mediastinale Lymphknotendissektion
durchgeführt werden. Die thoraxchirurgischen Standardresektion können mit verschiedenen
technischen Verfahren konventionell (offen-chirurgisch) oder minimal-invasiv (videoassistierte
thorakoskopische Operation (VATS)) durchgeführt werden.
The standard operating procedures are segmental, lobectomy including their modifications
(sleeve resection), bilobectomy and pneumonectomy. The decision on the extension of
