Die adäquate Versorgung der komplexen proximalen Humerusfraktur stellt noch immer eine Herausforderung innerhalb der Unfallchirurgie dar. Kaum eine Gelenkfraktur des Menschen wird hinsichtlich ihrer Therapie noch immer derart kontrovers diskutiert, was sich insbesondere an der regen Forschungstätigkeit in diesem Bereich der vergangenen Jahrzehnte widerspiegelt. Dabei existieren im Gegensatz zu anderen Frakturen des Bewegungsapparates keine exakten Behandlungsalgorithmen, welche, sofern konsequent eingehalten, zuverlässig vorhersagbare Ergebnisse produzieren könnten.

Das Ziel dieses Artikels soll sein, die vorhandenen Indikationen zu überprüfen und die Möglichkeiten, aber auch die Grenzen der konservativen Therapie auszuloten und vor dem Hintergrund der aktuellen Datenlage zu diskutieren. Ferner werden Art und Weise der weiteren ambulanten Begleitung des konservativ behandelten Patienten sowie Techniken der Ruhigstellung dargestellt.
