    Introduction

Since 2005, the Floating Mass Transducer (FMT) of the Vibrant Soundbridge is successfully coupled to the round window. Audiological and clinical long-term data – implanted 5 years or longer – was analyzed, firstly) to investigate the first available RW-Coupler, and secondly to evaluate the safety of the RW-coupling.

    Material and Methods

Data was available for 60 patients (65 ears). Investigated appointments were 3 months (3M) and 5 years (5Y), considering average air and bone conduction thresholds PTA (PTAAC,BC) and the word recognition score (Freiburg monosyllables at 65 dB SPL presentation level), as well as revision and explantation rates.

    Results

45 Data sets were available (30 without coupler, 15 RW-coupler) at the 5 years follow up appointment. Mean age of the patients was 57 years at surgery. Seven patients were explanted before 5Y (10, 8 %, not device or surgery related), the other patients were lost to follow up. The PTAACpreOP was 82.2 dB HL and the PTA BCPreOP 37.8 dB HL. Postoperative BC thresholds shifts from PTABC3M  = 37.3 dB HL to PTABC5Y  = 41.4 dB HL were not clinical relevant and not statistically significant. The median WRS was stable at both postoperative appointments (WRS3M=85, WRS5J=80), with WRSpreOP of 0% (n=24).

    Conclusion

Patients with a high air bone gap could be treated successfully with the Vibrant Soundbridge coupled to the round window, and even after 5 years good long-term audiological results were achieved.
