Zusammenfassung
Die atraumatische Sakrumfraktur (Sakruminsuffizienzfraktur) wurde bereits 1982 beschrieben.
Dennoch hat sich aufgrund der häufig schwierigen Diagnose, der überwiegend unspezifischen
Klassifikationssysteme und der vielfältigen Therapieoptionen bisher kein standardisierter
Behandlungspfad etabliert. Elementar ist zunächst die Unterscheidung der atraumatischen
Frakturen in primäre und sekundäre Frakturformen. Die konservative Therapie ist bei
der primären atraumatischen Sakrumfraktur (typischerweise osteoporotischen Insuffizienzfraktur
des Sakrums) bei neurologisch intakten Patienten in nahezu allen Fällen möglich und
erfolgsversprechend. Vorrangig sind dabei die Osteoporosetherapie und die adäquate
Analgesie zur Mobilisation sowie schließlich zur Rehabilitation unserer Patienten.
In den wenigen Fällen in denen die betagten Patienten von den genannten konservativen
Maßnahmen nicht profitieren, ist die minimal-invasive Therapie der Sakroplastie als
erweitertes schmerztherapeutisches Verfahren zu erwägen.
Abstract
The atraumatic fracture of the sacrum (sacral insufficiency fracture) was first described
as a clinical entity in 1982. However, due to frequently vague clinical symptoms and
difficult diagnosis, the predominantly nonspecific classification systems and the
various therapy concepts, no standardized treatment has yet been established. The
distinction between primary and secondary fractures is of essential importance within
the atraumatic fractures. Conservative treatment is promising in almost every case
of atraumatic sacral fracture (typically osteoporotic insufficiency fracture of the
sacrum) provided that the patient is neurologically intact. Priority needs to be given
to osteoporosis therapy and adequate analgesia for ambulation as well as to rehabilitation
of our patients. Sacroplasty should be considered as a minimally invasive alternative
for pain control in patients who do not benefit from conservative measures.
