Zusammenfassung

Kardiovaskuläre Symptome treten im klinischen Verlauf bei fast allen Patienten mit einem Parkinson-Syndrom auf. Sie können in allen Phasen der Erkrankung beginnen und unterschiedliche Schweregrade aufweisen. Die Symptome sind häufig unspezifisch und nicht richtungsweisend. Klinisch am bedeutendsten ist die orthostatische Hypotonie, die sich als anhaltender Blutdruckabfall über Minuten zeigt. Diagnostisch ist der Schellong-Test etabliert und sinnvoll. Die Therapie besteht aus konservativen Maßnahmen wie Stützstrümpfen. Medikamentöse Ansätze wie Midodrin sind nur bedingt hilfreich. Eine weitere wichtige klinische Auffälligkeit ist der fehlende Blutdruckabfall in der Nacht (Non-Dipper). Diagnostiziert wird dies mit einer 24-Stunden-Blutdruckmessung. Bei der Blutdruckeinstellung ist diese klinische Besonderheit zu beachten. Die MIBG-Szintigrafie ist eine wichtige Untersuchung, mit der zwischen einem idiopathischen und einer Multisystematrophie differenziert werden kann. Sie belegt darüber hinaus eine Mitbeteiligung des peripheren autonomen Nervensystems bei der Erkrankung.

Abstract

Cardiovascular symptoms occur in the clinical course in almost all patients with Parkinsonʼs disease. They can begin in all phases of the disease and have different degrees of severity. The symptoms are often unspecific. Clinically most significant is the orthostatic hypotonia, which shows itself as a persistent blood pressure drop over minutes. Diagnostically, the Schellong test (lying-to-standing orthostatic test) is established and meaningful. The therapy consists of conservative measures such as stockings. Pharmaceutical approaches such as midodrin are only partly helpful. Another important clinical manifestation is the lack of blood pressure drop at night (non-dipper). This is diagnosed with a 24-hour blood pressure measurement. In the case of blood pressure adjustment, this clinical peculiarity must be observed. MIBG scintigraphy is an important examination that can differentiate between idiopathic and multi system atrophy. It also demonstrates the involvement of the peripheral autonomic nervous system in the disease.

Was ist wichtig?

Beim idiopathischen Parkinson-Syndrom treten häufig kardiovaskuläre Symptome auf, die in ihrer Differenzierung, aber auch in der Therapie Schwierigkeiten bereiten. Hervorzuheben sind dabei folgende 4 Aspekte:

  • Das Parkinson-Syndrom ist eine Multisystemdegeneration, bei der das zentrale und periphere autonome, kardiovaskuläre System beteiligt sind.

  • Besonders häufige Symptome sind die orthostatische Hypotonie und die fehlende Nachtabsenkung des Blutdrucks.

  • Parkinson-Patienten haben daneben kardiovaskuläre Erkrankungen in ähnlicher Häufigkeit wie eine alterskorrigierte Vergleichspopulation.

  • Etliche Medikamente, die beim Parkinson-Syndrom eingesetzt werden, können zu kardiovaskulären Symptomen führen.

Medikamente, die wegen kardiovaskulärer Symptome eingenommen werden, können bei Parkinson-Patienten eine orthostatische Hypotonie verstärken. Besondere Vorsicht gilt bei Betablockern und Diuretika.
