Dentinogenic ghost cell tumors (DGCTs) are rare benign odontogenic neoplasms, with less than 100 cases reported in the literature. These tumors normally present in the mandible or maxilla; and while they are classified as benign, they have the potential to be very locally aggressive, with even some cases demonstrating malignant transformation. This case report follows a 24-year-old male with a recurrent DGCT of the right maxilla who presented with massive facial deformity with erosion of his right maxilla, facial skin, right orbit, nasal dorsum, and anterior cranial fossa. This case report details this unusual tumor with malignant transformation and rapid growth, necessitating multidisciplinary coordination and complex composite craniofacial resection with subsequent free tissue transfer for reconstruction. We also review the literature with respect to these rare cases and discuss future considerations for investigation.
