Olfactory groove meningiomas (OGMs) can be approached with an endoscopic endonasal approach (EEA). Despite advantages of EEAs, disadvantages are anosmia and removal of the ostiomeatal complex—which predisposes the patient to colder, nonphysiological nasal airflow and consequent nasal crusting. To mitigate removal of healthy nasal structures, we present an OGM removed using the superior ethmoidal approach, which preserves both middle turbinates and the middle ostiomeatal complex and results in more physiological nasal airflow.
