Introduction Large scalp defects represent a surgical challenge. Despite numerous reconstructive options, there is still a risk of wound dehiscence or even necrosis. The individual patient's will and general condition must also be taken into account.

Material Using the example of an 86-year-old male patient, defect reconstruction using dermal regeneration template (Integra) will be demonstrated.

Conclusions Dermal regeneration templates expand the portfolio of reconstructive options. Their use represents a good alternative to local flap plasty, particularly in the case presented.
