Fenestrated-Branch Endovascular Repair (FEVAR) for Para-anastomotic Huge Aortic Pesudoaneurysm

Case Presentation: Para-anastomotic pseudo-aneurysm after open abdominal aortic repair is a rare but fatal complication. Management is difficult and more challenging because of visceral and renal arteries involvement. A 66-year-old man admitted 10 years after open surgical repair of abdominal aortic aneurysm. History of MI and CABG 11 years ago. Patient has ICD with LVEF 20%. The aortic pseudo-aneurysm was 57 mm and standard EVAR or even double CHEVAR not possible due to juxta-renal pathology and short landing zone to SMA. T-Branch BEVAR was not possible because of small internal luminal diameter in visceral aorta. The pseudo-aneurysm was managed successfully with Four-fen fEVAR extended to the abdominal previous tube graft and the patient achieved a good recovery. Conclusion: Fenestrated endo vascular aortic repair after prior open surgical repair is associated with low peri-operative morbidity and mortality, and acceptable medium term survival.
