Cutaneous leiomyosarcoma is a neoplasm most commonly found in the lower limbs, torso and extremities, but it is a rare tumor on the face, with less than 1-5% of skin and smooth muscle neoplasms. Its involvement is rare, representing 5% within cutaneous leiomyosarcomas. This work reports a case of a patient with recurrent face leiomyosarcoma, with nodular, ulcerated, vegetative lesion with pale mucosa staining at the site of the maxillary 7,0cm operative wound, with spontaneous bleeding and absence of cervical adenopathy. Because it has a nonspecific clinic, the diagnosis is made through immunohistochemistry and anatomopathologica, since there are no literature describing the effectiveness of imaging tests for staging and diagnosis. It has high rates of local recurrence, as in this case, but with low mortality and metastasis rates. Mohs micrographic usually is the best surgery technique due to the low local recurrence rates after removal of the lesion. Specifically, the treatment was performed using radiotherapy associated with wide and deep excision to avoid the risk of relapses, which usually are more aggressive, extending to the subcutaneous and increasing the risk of metastasis.
