ECCRINAL CARCINOMA OF THE VULVA :CASE REPORT AND BRIEF LITERARY REVIEW

    Primary malignant tumors of the vulva represent only 0,6% of all cancers in women. Squamous cell Carcinoma is the most common histological type, representing 90% of all cases. Eccrinal Carcinoma is a rare malignant tumour that grows from sweet glands, and its estimated incidence is 0,005 to 0,01% of all cutaneous tumours. We present a case of a 60 year old woman with a 6cc vulvar tumour , near the right vaginal introitus, invading the vaginal wall and pelvic bone structures, growing for at least 1 year with local pain ( T3N0M0, FIGO IVA). Histopathological examination of the original biopsy revealed an undifferentiated infiltrative carcinoma. Treatment included neoadjuvant radiochemoterapy followed by radical vulvectomy. Imunohistochemical panel from the vulvectomy specimen defined the Eccrinal carcinoma diagnosis. The patient is now 2 years in following, with no apparent recurrence. Discussion: A review of the literature showed 9 cases of vulvar eccrinal carcinoma described in the english language. An analysis of all these patients , including the one presented, revealed that the age range for these patients was from 32 to 88 years, with an average of 65.3 years. The location of the tumour was most common on the Labia majora. The size at diagnosis varied from 2 to 5 cm ( average of 3,4 cm). Considering the low prevalence of eccrinal carcinomas, the diagnosis of this tumour can be challenging. Follow up monitoring varied from 6 to 132 months ( average of 35.1 months) Two (20%) recurred, 3(30%) presented metastasis to regional lymph nodes, and 2 (20%) metastasis to other sites. Only one patient died form the disease, 3 remain alive with recurrence and 4 with no evidence of tumour. Final commentaries: Although it is a rare entity sweat gland carcinoma should be considered during differential diagnosis of vulvar masses. In vulvar cases it is difficult to establish prognostic predictors fo Eccrinal Carcinoma. However, aggressive signs in extra genital cases may be applied in vulvar tumours
