Introduction: The recent spread of the transorbital endoscopic approach has introduced a new strategy for the treatment of orbital pathologies. Its use, either as an alternative to or in combination with the transnasal endoscopic approach, allows for comprehensive management of orbital lesions from all angles. The aim of this study is to present our experience—in terms of clinical and radiological outcomes—in the treatment of orbital cavernous hemangiomas using endoscopic approaches.

Materials and Methods: We retrospectively retrieved and analyzed data from patients treated for orbital cavernous hemangiomas with endoscopic approaches at three Italian tertiary referral centers between 2016 and 2023.

Results: Our surgical series of patients operated on for the removal of orbital cavernous hemangiomas using endoscopic approaches includes 34 patients. In all cases, GTR (gross total resection) was achieved, and no postoperative complications were recorded, except for palpebral edema—which always resolved within a few days—in some cases n some cases within the transorbital approach group

Conclusion: endoscopic approaches provide an excellent option for the treatment of orbital cavernous hemangiomas.
