This video details an endoscopic Denker’s maxillectomy approach to resection of a trigeminal schwannoma. This technique has previously been described for the resection of lesions in the maxillary sinus, pterygopalatine, and infratemporal fossae. Here we present a novel use which includes a transpterygoid dissection to reach Meckel’s cave within the middle cranial fossa. This approach allowed for access to the lateral aspect of the mass, as well as exposure of the internal carotid artery.
