Orbitopterional Craniotomy, A3-A3 Bypass, and Clip Trapping of Giant Unruptured Anterior Communicating Artery Aneurysm
We present the case of a 35-year-old male who presents with bitemporal hemianopsia with a giant, partially thrombosed anterior communicating artery aneurysm. The patient underwent an orbitopterional craniotomy, A3-A3 bypass, and clip trapping. Preoperative balloon test occlusion as well as intraoperative angiography was used in decision making to determine the need for bypass. Complete aneurysm obliteration was confirmed on postoperative angiography. The patient was discharged home with improvement from his preoperative vision baseline.
