Effektivität und Anwendbarkeit neuropsychologischer Rehabilitationsverfahren[*]

Zusammenfassung:

Etwa eine halbe Million Menschen in Deutschland werden jedes Jahr mit einer der Hauptdiagnosen zerebrovaskuläre Erkrankung oder Schädel-Hirn-Trauma stationär behandelt. Auch nach der Entlassung aus der Akutklinik zeigt eine große Anzahl von ihnen neuropsychologische Beeinträchtigungen der Aufmerksamkeit, des Gedächtnisses, der Sprache oder der Wahrnehmung. Neuropsychologische Trainings werden fast ausschließlich im stationären Rehabilitationssystem vorgehalten, das im Wesentlichen von den Rentenversicherungsträgern finanziert wird. Eine solche stationäre Rehabilitationsmaßnahme erhalten weit weniger als 5 % dieser Patienten, ambulante neuropsychologische Programme gibt es hier nicht. Im deutschen System ist deshalb die Frage besonders wichtig, wie effektiv diese Einzeltrainings tatsächlich sind, und vor allem, ob und wie Erfolge dort auf Alltagsverhalten generalisieren. Studien zu dieser Thematik aus den letzten zehn Jahren werden analysiert und diskutiert. Nach wie vor genügen wenige Studien durchweg den generellen Anforderungen, die Ergebnisse sind uneinheitlich, aber doch so, dass es sinnvoll scheint, neuropsychologische Rehabilitation häufiger anzubieten, was zudem weitere Evaluationsmöglichkeiten bieten würde.

Effectiveness and Usefulness of Neuropsychological Rehabilitation Approaches:

Every year, more than half a million people in Germany experience a stroke or a traumatic brain injury. A high percentage of them leave the hospital with neuropsychological deficiencies of attention, memory, speech or perception preventing them from returning to work. The German rehabilitation system is primarily funded by the statutory pension insurance system and is hospital-based, but only very few brain-damaged patients have the opportunity to be treated there. Hospital rehabilitation programmes often integrate neuropsychological trainings whereas neuropsychologically based outpatient programmes do not exist. Therefore two questions are crucial. First, are those separate neuropsychological trainings effective? Second, are positive effects generalized? Studies concerning these questions are reviewed. Not many of them, however, meet the usual requirements for valid intervention research, and their findings are equivocal. Nevertheless, they suggest that neuropsychological rehabilitation should be undertaken more frequently, which would offer a framework for future efficacy research.
