Zusammenfassung.
Die Linderung von Beschwerden von Krebspatienten ist zentrales ärztliches Anliegen. Das WHO-Programm zur Bekämpfung des Schmerzes basiert auf dem Prinzip eines 3-Stufen-Schemas, in dem ansteigender Schmerz durch stufenweise stärkere Analgetika behandelt werden. Das Schema berücksichtigt weder Chemotherapie, chirurgische Interventionen, neue pharmakologische Ansätze noch psychologische und radiologische Therapieformen zur Analgesie. Es ist daher nicht unumstritten. Wir schlagen ein multidimensionales Modell auf der Basis einer Pyramide vor, durch das die Beschwerdelinderung von Krebspatienten besser konzeptionell wiedergegeben wird. Hierdurch wird sowohl die Komplexität von Symptomen berücksichtigt als auch der gegebenen Multidisziplinarität und Multiprofessionalität in der Behandlung von Beschwerden Rechnung getragen. Das Pyramidenmodell repräsentiert darüber hinaus einen konzeptionellen Ansatz, durch den sich der Therapeut dem Patienten diagnostisch nähert. „Metapyramiden” sind neue Pyramiden, die aus jeder Seite der Ursprungspyramide entstehen können und zu einer jeweils tieferen Detailebene hinführen. Das Pyramidenmodell kann leicht auf alle möglichen Symptome übertragen werden. Wir stellen vier Möglichkeiten dar, wie das neue Pyramidenmodell validiert werden kann, um es akzeptabel, wissenschaftlich solide und klinisch nutzbar zu machen.
A New Paradigm for Cancer Symptom Control - the Pyramid Approach.
Cancer symptom control is a medical issue which has become a public health challenge in the 21 St century. The WHO Cancer Pain programme had as its central principle, the three-step analgesic ladder, in which pain of increasing severity is tackled by drugs (predominantly opioids) of increasing potency. This approach does not take into account the useful contributions to pain control from anti-cancer treatments, surgical interventions, new pharmacological approaches, and the psychological and complementary therapies. We propose a new multidimensional model - based on a pyramid - in which cancer symptom control is approached from different directions, accounting for true multimodality and multidisciplinary management. This model can also be used to explain a more holistic approach to patient assessment including investigation, treatment and evaluation. “Meta-pyramids” are new pyramids derived from each face of a pyramid, which lead to even deeper levels of detail. The pyramid model can be readily adapted to symptoms other than pain. We propose four ways of validating the new pyramid model in terms of acceptability, scientific rigour and clinical usefulness.
