Zusammenfassung
Die Extrakorporale Stoßwellentherapie (ESWT) wird derzeit in der konservativen Therapie
schmerzhafter Erektionen bei Induratio Penis Plastika (IPP) zunehmend und mit Erfolg
eingesetzt. Andere Symptome der IPP, wie Abknickung des Gliedes oder Erektionsstörungen
lassen sich nicht so günstig beeinflussen. Die hier dargestellten Daten von 74 mit
ESWT behandelten Patienten bestätigen dies. Eine zusätzliche lokale Injektion eines
Ca-Antagonisten (Verapamil) in den Plaque, welche bei 17 der 74 Patienten in einer
zweiten Behandlungsserie durchgeführt wurde, erbrachte eine weitere geringfügige Verbesserung
der Schmerz-Symptomatik sowie palpatorisch weicher werdende Plaques. Bei Deviation
und Erektionsstörungen traten jedoch keine Besserungen auf, so dass auch für die Kombinationsbehandlung
die Indikation bei vorherrschender Schmerz-Symptomatik bei IPP gesehen wird.
Abstract
In the conservative treatment of painful penile erection due to Peyronie's Disease
there is a trend towards extracorporal shockwave therapy (ESWT). Other symptoms of
Peyronie's Disease like penile angulation or erectile dysfunction cannot be influenced
in that amount. This can be confirmed by the here presented data of 74 patients treated
by ESWT. Additional local injections of a calcium-antagonist (Verapamile) into the
plaque, performed in 17 of the 74 patients in a second course of therapy, achieved
little improvement of the pain and a softening of the plaques. Angulation and erectile
dysfunction were not improved. We conclude, that a combination of ESWT and local injections
is limited to the therapy of painful penile erections in Peyronie's Disease.
