Summary

Ascariasis is the most common intestinal helminthiasis worldwide. Heavily infected individuals are prone to develop bowel obstruction or perforation as well as biliary disease. Nevertheless, the presence of roundworms in the biliary tree outside endemic areas is very uncommon. The migration of these worms to the biliary system can cause biliary colic, pancreatitis, or even acute suppurative cholangitis with hepatic abscesses and septicemia.

We report here on 2 infants with 14 and 15 months and a 9-year-old boy who suffered from massive biliary ascariasis and who presented with acute suppurative cholangitis. All cases were successfully treated by endoscopic retrograde cholangiopancreatography with worm extraction and adjuvant medical therapy.

Physicians should be aware of ascariasis in patients with pancreatobiliary symptoms who have traveled to endemic areas or in immigrants from these areas.

Die Infektion mit Ascaris lumbricoides ist weltweit die häufigste intestinale Helminthose. Massiv infizierte Individuen sind sowohl gefährdet, eine Darmobstruktion oder -perforation als auch biliäre Beschwerden zu bekommen. Außerhalb von Endemiegebieten ist das Vorkommen von Askariden im biliären Gangsystem ungewöhnlich. Die Migration der Würmer im Gallengangssystem kann biliäre Koliken, eine Pankreatitis, aber auch eine akute eitrige Cholangitis mit Leberabszessen und im Einzelfalle ein septisches Krankheitsbild auslösen.

Wir berichten über 2 Kinder im Alter von 14 und 15 Monaten und einem 9-jährigen Jungen, die an einer massiven biliären Askariasis litten und sich unter dem klinischen Bild der akuten eitrigen Cholangitis präsentierten. Alle Fälle wurden erfolgreich mittels endoskopisch-retrograder Cholangiopankreatikographie (ERCP) mit Wurmentfernung und adjuvanter medikamentöser Therapie behandelt.

Im Zeitalter des weltweiten Tourismus sollten Ärzte bei Patienten, die sich in einem entsprechenden Endemiegebiet aufgehalten und pankreatobiliäre Beschwerden haben, auch an das Vorliegen einer Askariasis denken.
