Spermatic Cord Torsion After Testicular Fixation

Summary

We present a technique for testicular fixation, based on the development of a new anatomical structure formed exclusively by the visceral and the parietal tunica vaginalis; our pre-clinical experience with this technique performed in guinea pigs and a clinical casuistic of 100 operations in 69 patients (age range: newborns to 25 years, mean 101.43 months) where the principal indications were unilateral torsion, testicular descent and contralateral fixation after orchiectomy. Postoperative follow-up ranged between 4 months and 204 months with a mean of 14.304 months. At their final follow-up, all patients presented normal testicular size, shape, consistency, sensitivity, mobility and intrascrotal position; no patient complained of symptoms related to the operation. The discussion also includes 48 cases of failed testicular fixation which were performed with sutures, vaginal adhesions or dartos-pouch techniques, as reported by 30 different authors. Ages of patients at the primary operation ranged from 3 years to 34 years (mean 14.381 years), at the secondary operation from 10 years to 40 years (mean 19.298 years) and the interval between them ranged from 5 days to 27 years (mean 53.773 months). The causes of the failures, which led us to consider them as unreliable, were absorption of cicatricial tissue, serosal regeneration and loosening of sutures. The sequelae included testicular atrophies, orchiectomies and severely damaged testicles.

Résumé

Les auteurs présentent une technique pour fixer les testicules, fondée sur la réalisation d'une nouvelle structure anatomique constituée exclusivement par la vaginale, pariétale et viscérale. Une expérimentation a été réalisée sur des cobayes. Une étude clinique portant sur 100 interventions réalisées chez 69 patients (du nouveau-né à l'âge de 25 ans, âge moyen: 101,43 mois). Les indications principales étaient torsion unilatérale, et fixation controlatérale descente testiculaire après ablation testiculaire. Les contrôles post-opératoires ont été réalisés entre 4 mois et 204 mois (moyenne: 14,304 mois). Au moment du bilan final, tous les patients montraient des testicules normaux, en ce qui concernent leur dimension, forme, consistance, sensibilité, motilité et position intra-scrotale. Aucun des patients ne présentait de symptômes liés à l'intervention. Les discussions comportent aussi 48 cas d'échec de fixation testiculaire réalisées par sutures, adhésions vaginales ou poche dans le dartos; l'âge des patients au moment de la première opération (de 3 ans à 34 ans, moyenne 14,381), au moment de la seconde opération (de 10 ans à 40 ans, moyenne 19,298) et l'intervalle entre les deux (de 5 jours à 27 ans, moyenne 53,773 mois).

Les causes des échecs sont en rapport avec la résorption du tissu cicatriciel, la régénération de la séreuse et la disparition des sutures. Les séquelles incluent des atrophies testiculaires, des orchidectomies et des testicules sévèrement altérés.

Resumen

Presentamos una técnica para fijación testicular basada en la formación de una nueva estructura anatómica formada exclusivamente por la tunica vaginalis parietal y visceral; nuestra experiencia pre-clínica con esta técnica en cobayos y una casuística clínica de 100 operaciones en 69 pacientes (edad: rango recién nacidos hasta 25 años, media 101.43 meses) cuyas las indicaciones principales fueron torsión unilateral, descenso testicular y fijación contralateral post orquiectomía. Se realizaron controles postoperatorios entre 4 meses y 204 meses - media 14,304 meses; en el control final todos los pacientes presentaron testículos normales en cuanto a tamaño, forma, consistencia, sensibilidad, movilidad y posición intraescrotal; ningún paciente presentó síntomas vinculados a la operación. Se discuten asimismo 48 casos de fallas en fijaciones realizadas con suturas, adherencias intravaginales o el bolsillo del dartos, descriptas por 30 autores distintos. La edad de los pacientes en la operación primaria fue de 3 años a 34 años, media 14,381 años, en la secundaria 10 años a 40 años, media 19,298 años y el intervalo entre ambas 5 días a 27 años, media 53,773 meses. Las causas de las fallas que nos llevaron a considerarlas como no confiables son absorción de tejido cicatrizal, restitución de continuidad de la serosa y liberación de suturas. Las secuelas de estas fallas incluyen atrofias testiculares, orquiectomías y testículos severamente dañados.

Zusammenfassung

Es wird eine Technik der Hodenfixation vorgestellt, die auf der Bildung einer neuen anatomischen Struktur zwischen Tunica vaginalis visceralis und parietalis besteht. Zunächst wurde die beschriebene Technik an Meerschweinchen erarbeitet und dann bei 100 Eingriffen an 69 Patienten im Alter vom Neugeborenen bis zu 25 Jahren (Mittelwert: 101,43 Monate) überprüft. Hauptindikation der Orchidopexie bei diesen Patienten war eine unilaterale Hodentorsion, ein Maldescensus testis oder eine Fixation des kontralateralen Hodens nach Orchidektomie. Die postoperative Nachkontrolle erfolgte zwischen 4 und 204 Monaten (Mittelwert: 14,304 Monate). Bei der letzten Kontrolluntersuchung zeigten alle Patienten eine normale Hodengröße, Hodenform, Konsistenz, Empfindlichkeit und Beweglichkeit sowie unauffällige intraskrotale Lage. Darüber hinaus werden 48 Fälle von Fehlfixation diskutiert, bei denen die Orchidopexie durch intravaginale Verklebungen oder Verlagerung des Organs in eine Shoemaker-Tasche erfolgte. Das Alter dieser Patienten mit bisher üblicher Orchidopexie lag zwischen 3 und 34 Jahren (Mittelwert: 14,381 Jahre). Bei dem zweiten Eingriff waren die Patienten 10 - 40 Jahre alt (Mittelwert: 19,298 Jahre, der Zeitraum zwischen den Eingriffen betrug 5 Tage bis 27 Jahre, Mittelwert 53,772 Monate). Die Gründe, die bisher übliche Fixationsmethode zu verlassen, sind Absorptionen von Narbengewebe, Regenerierung der Serosa und Nahtlösungen. Die Folge waren Hodenatrophien, die wiederum zur Orchidektomie führten und schwere Schädigungen der Keimdrüse.
