Zusammenfassung
Eine 45-jährige Hausfrau wurde in der 22. Schwangerschaftswoche mit Unterleibsschmerzen
eingeliefert. Die Patientin gab an, dass der Schmerz vor fünf Tagen, nach einem Sturz,
eingesetzt und sich während der letzten beiden Tage verschlimmert habe und wirkte
ängstlich und erregt. Die klinische Untersuchung ergab die tastbare Spitze eines metallischen
Fremdkörpers an der vorderen Abdominalwand. Der sonographische Befund war eine normale
Schwangerschaft in Übereinstimmung mit den Amenorrhödaten. Die Plazenta war in vorderer
Position und der Aspekt des Fötus normal. Ein schmaler metallischer Gegenstand befand
sich entlang der vorderen Uteruswand. Laparotomie der unteren Hälfte zeigte eine Perforation
des Uterus mittels einer 7 cm langen Stricknadel. Es erfolgte eine Hysterektomie mit
linksseitiger Adnexektomie, weil keine Hämostase erreicht werden konnte. Es wurden
eine antibiotische Behandlung und Tetanusprophylaxe eingeleitet. Der Eingriff war
postoperativ ereignislos. Die Patientin wurde in gutem Zustand am 13. postoperativen
Tag entlassen. Der hinzugezogene Psychiater beurteilte die Patientin als „einfache
Persönlichkeit“ mit psychogenen Reaktionen.
Abstract
A 45-year-old housewife was admitted in the 22nd week of gestation because of abdominal
pain. The patient stated that the pain began after a fall five days previously and
increased during the last two days. The patient was anxious and agitated. Clinical
examination showed the palpable tip of a metal foreign body on the anterior abdominal
wall. Ultrasonography showed a normal pregnancy consistent with the dates of amenorrhea.
The placenta was situated anteriorly and the fetus appeared normal. A thin metal object
was seen along the anterior uterine wall. Lower midline laparotomy revealed uterine
perforation by a 7-cm knitting needle. Hysterectomy with left-sided adnexectomy was
performed because hemostasis could not be established. Antibiotic treatment and tetanus
prophylaxis were administered and the postoperative course was uneventful. The patient
was discharged from the hospital in good condition on the 13th postoperative day.
A psychiatrist described a simple personality with psychogenic reactions.
