Zusammenfassung

Einleitung: Obwohl Männer mit Klinefelter-Syndrom (KS) vom Nicht-Mosaik-Typ (47,XXY) normalerweise unter einer Azoospermie leiden und bisher als infertil betrachtet wurden, belegten neuere Forschungen, dass Spermatogonien mit dem Karyotyp 47,XXY zur vollständigen Spermatogenese fähig sind und durch ICSI fortpflanzungsfähig sind. Kasuistik: Bei den Ehemännern von zwei Ehepaaren mit primärer androgener Infertilität der Ehemänner aufgrund einer sekretorischen Azoospermie bei einem KS vom Nicht-Mosaik-Typ (47,XXY) erfolgte die testikuläre Spermienextraktion. Beide Ehefrauen hatten einen normalen Karyotyp und wiesen keine pathologischen Befunde in der gynäkologisch-endokrinen Diagnostik auf. ICSI wurde durchgeführt nach ovarieller Stimulation und transvaginaler, ultraschallgesteuerter Oozytenaspiration mit anschließender Injektion von testikulär akquirierten Spermatozoen. Es erfolgten eine reguläre Implantation und normale Schwangerschaft in beiden Fällen. Die genetische Analyse aus peripherem Blut der Neugeborenen zeigte einen normalen 46,XX- bzw. 46,XY-Karyotyp. Schlussfolgerung: Diese Fallbeispiele untermauern die These, dass Patienten mit KS vom Nicht-Mosaik-Typ fähig sind, normale Spermatozyten mit normalem Fertilisationspotenzial zu bilden. Auch wenn es aufgrund bisher nur geringer Fallzahlen in der Literatur verfrüht erscheint, Rückschlüsse bezüglich der Transmissionsrate dieser Aneuploidie zu ziehen, zeigen doch diese beiden Fälle, dass mit einer eher geringen Transmissionsrate dieser gonosomalen Aneuploidie bei Männern mit KS vom Nicht-Mosaik-Typ, die sich der In-vitro-Fertilisation mit ICSI unterziehen, zu rechnen ist.

Abstract

Introduction: Although men with chromosomal non-mosaic Klinefelter's syndrome (47,XXY) present with azoospermia and have been considered infertile in the past, recent findings suggest that 47,XXY spermatogonia are able to undergo and complete spermatogenesis. Non-mosaic Klinefelter's men may even reproduce through intracytoplasmatic sperm injection (ICSI). Case Report: Two couples with primary infertility and husband's secretory azoospermia and non-mosaic 47,XXY karyotype underwent intracytoplasmatic sperm injection (ICSI) combined with testicular sperm extraction. Both wives had a normal karyotype without any gynecological pathology. The women conceived and normal pregnancy followed. Genetic analysis of the neonates revealed a normal 46,XX and 46,XY karyotype, respectively. Conclusions: These case reports reaffirm the thesis that the testes of men with non-mosaic Klinefelter's syndrome is capable of producing normal spermatozoa with fertilization potential. Although it is not possible to draw conclusions about the rate of transmission of this aneuploidy, because of the low number of published cases, the birth of two healthy babies after in vitro fertilization plus ICSC of testicular spermatozoa from two non-mosaic men with Klinefelter's syndrome strongly suggests that low transmission of this gonosomal aneuploidy can be expected.
