Obwohl Vitamin D seit über 200 Jahren in Form von Fischleber-Ölen als ein Volksheilmittel im Gebrauch ist, besteht Unklarheit darüber, welche Dosis ausreichend ist. Die für Kinder etablierte Dosisempfehlung (400IE/d) entstammt dem Konzept der Vermeidung einer Rachitis und beruht auf der Äquivalenzdosis eines Teelöffel Lebertrans. Die noch weitgehend gültigen Empfehlungen für Erwachsene (von 200IE/d) beruhen relativ willkürlich auf der Hälfte dessen [57].
Neue Erkenntnisse weisen jedoch daraufhin, dass ein adäquater Vitamin-D-Status nicht nur zur Vermeidung von Rachitis oder Osteomalazie essentiell ist, sondern auch positive Einflüsse auf eine Vielzahl von Organfunktionen, insbesondere Muskelkraft, Vermeidung von Autoimmunerkrankungen sowie Krebsabwehr hat, wobei hierfür möglicherweise höhere Spiegel notwendig sind. Messungen des Anstiegs des 25-OH-Vitamin-D-Spiegels unter Sonnenexposition sowie im Vergleich unter Cholecalciferol- und Ergocalciferolgabe weisen darauf hin, dass die zur Substitution bei fehlender Sonnenexposition verbreiteten Dosisempfehlungen wesentlich zu niedrig und niedrige 25-OH-Vitamin-D-Spiegel sehr häufig sind. Vitamin-D-Mangel ist wahrscheinlich der häufigste Vitaminmangelzustand überhaupt, dessen Verbreitung im Übrigen epidemische Ausmaße zeigt [21] .
