Severe Febrile Reactions after Therapy with Oxaliplatin of Metastatic Colorectal Cancer

Zusammenfassung

Oxaliplatin hat als neues Medikament in der Therapie des fortgeschrittenen kolorektalen Karzinoms aufgrund seines günstigen Nebenwirkungsprofils bei nachgewiesen überlegener Wirksamkeit gegenüber einer Monotherapie mit Fluorpyrimidinen, einen festen Stellenwert. Üblicherweise wird es in Kombination mit einem infusionalen 5-FU-Protokoll über ein venöses Portsystem appliziert. In unserem Fall berichten wir über differenzialdiagnostische Probleme bei einer Patientin mit febrilen Reaktionen unter einer Oxaliplatintherapie, die das Bild septischer Episoden imitierten. Aufgrund der relativen Häufigkeit febriler Episoden unter Oxaliplatin muss neben einer septischen Genese durch ein infiziertes Portsystem immer auch an eine allergische Reaktion auf das applizierte Medikament gedacht werden.

Abstract

Oxaliplatin has gained acceptance in therapy of metastatic colorectal cancer because of its low toxicity profile and ist superior antitumor effect compared to therapy with fluoropyrmidines. Commonly Oxaliplatin is part of a combination regimen including infusional 5-FU, administered via an implantable port-system. In the case report presented, we describe diagnostic problems in a patient with fever under therapy with oxaliplatin, mimicking septic episodes. Because fever is a frequent side effect with Oxaliplatin treated patients, in case of fever an allergic reaction on Oxaliplatin has to be differentiated from septical infection of the port system.
