Zusammenfassung

„Colour Velocity Imaging (CVI)” ist eine Ultraschalltechnologie, mit der über qualitative Parameter (z. B. Pulsatilitäts- oder Resistance-Index) hinaus der Volumenfluss im fetalen Kreislauf quantitativ beurteilt werden kann.

In einer Machbarkeitsstudie wurden Volumenflüsse in der Nabelvene bei unauffälligen Feten zwischen 17 und 39 SSW untersucht.

Die Datenaufnahme für CVI ist schnell und einfach: Der Cineloop einer Sequenz über einige Herzzyklen wird binnen Sekunden auf den Rechner übertragen. Es besteht eine deutliche Intraobserver-Variabilität, die z. T. messbedingt (Winkelabhängigkeit), aber auch durch physiologische Variation bedingt sein kann.

Die Messung des fetalen Volumenflusses z. B. mittels CVI ist eine neue Methode zur quantitativen Analyse des fetalen Kreislaufes. Die Berechnungen beruhen auf angenommenen Standardbedingungen (laminarer Fluss, runder Gefäßquerschnitt). Fetale Bewegungen, ein geschlängelter Gefäß verlauf oder ein nicht-kreisrunder Querschnitt sind mögliche Fehlerquellen.

Abstract

“Colour Velocity Imaging (CVI)” permits acquisition of ultrasound Doppler data for quantitative analysis. CVI can be used to study the fetal circulation. In a pilot study we attempted measuring umbilical venous flow.

Data acquisition is simple and fast. In the current implementation, data is sent to a work-station and analysed off-line. There is significant “intra-observer-variability” that could be attributable to true physiological variation or measurement inaccuracies.

Measurement of fetal umbilical volume flow by CVI is a new and simple technique. Before its clinical use assumptions about the vessel, flow, variability and normal values must be verified in the fetus.
