Zusammenfassung

Seit den 50er-Jahren des letzten Jahrhunderts ist weltweit ein Rückgang der Stillkultur zu verzeichnen. Dies führte in den 80er Jahren zu einem verstärkten Engagement der WHO und der UNICEF, da die Vorteile des Stillens für das Kind und Mutter evident sind. 1990 hat die UNICEF in den „Zehn Schritten zum erfolgreichen Stillen” weltweit den Entbindungskliniken Richtlinien an die Hand gegeben, die einen erfolgreichen Start in eine problemlosere Stillbeziehung ermöglichen sollen. Die Schritte können als Maßnahmen zur Motivationsförderung, Schulung und Instruktion des Personals und als Maßnahmen zur Motivation und der Unterstützung der Eltern zusammengefasst werden. Die weiteren 10 Schritte befassen sich mit dem optimalen Stillbeginn und es werden die Einflussfaktoren des Zufütterns dargestellt. Die Zertifizierung der Geburtskliniken durch international anerkannte externe Gutachter stellt eine Möglichkeit dar, die hohe Qualität der Stillförderung zu sichern und den Prozess in den Klinken zu fördern. Der positive Effekt der Richtlinien auf die Stillquote und auf die Gesundheit der Kinder konnte in randomisierten Studien nachgewiesen werden. Die Erfahrungen der Klinik für Gynäkologie und Geburtsmedizin des Vivantes Humboldt-Klinikums werden dargestellt.

Abstract

A decrease of breastfeeding culture could be observed since the 1950's. However in the 1980's the WHO and UNICEF began engaging in campaigns for breastfeeding since the benefits for mother and child are evident. In 1990 UNICEF set up the “10 steps to successful breastfeeding” that gave birth clinics around the world “guidelines” that would initiate an unproblematic breastfeeding relationship between mother and child. These guidelines can be understood as measures toward increasing staff motivation, -training and -instruction as well as actually helping parents and increasing their motivation. Further points cover the optimal beginning of breastfeeding and aspects of complementary feeding.
Certification of departments by internationally renowned observers is a means of implementing and securing best quality breastfeeding encouragement in the birth clinics.
The positive effects of this promotion on the ratio of mothers breastfeeding and thus in respect to children¿s health could be proved by randomised studies. The experience gained in the Obstetrical Department of the Vivantes Humboldt Clinic is described.
