Diagnose und Therapie von Schmerzverstärkungssyndromen bei Kindern: Erfahrung aus zwei Jahrzehnten[*]

Zusammenfassung

Kinder mit Schmerzverstärkungssyndromen leiden unter starken Schmerzen und Behinderungen und benötigen liebevolle Betreuung, die eine gezielte Diagnose beinhaltet, sowie eine konsequente Therapie. Psychologische Aspekte müssen angesprochen werden. Intensive Trainingstherapie und Desensitisierung lösen die Schmerzen bei den meisten Patienten. Bei anhaltenden Schmerzen muss die Funktion weiterhin aufrechterhalten werden und es muss eine gezielte Psychotherapie erfolgen. Medikamente helfen nicht. Sie sollten auf spezifische Indikationen beschränkt bleiben und nicht zur Schmerztherapie herangezogen werden. Kinder mit Schmerzverstärkung bedeuten eine große Herausforderung, aber die positiven Erfahrungen, die mit ihnen gemacht werden, sind eine echte Belohnung für alle Beteiligten.

Abstract

Children with amplified musculoskeletal pain suffer from severe pain and disability and need compassionate care that should include an accurate diagnosis as well as a dependable therapeutic approach. Psychological aspects should be formally addressed. Intense exercise therapy along with desensitization resolves all signs and symptoms in most patients. If pain persists, function still needs to be maintained and targeted psychotherapy should be pursued. Pharmacological agents do not help, and should be limited to specific indications and not given for pain control. Children with amplified musculoskeletal pain can be very challenging, but the benefits gained are a genuine reward for all participants.
