Einführung

Die Verwendung von Silikonimplantaten ist ein etabliertes Verfahren zur primären und sekundären Rekonstruktion nach abladierenden Operationen, zur Therapie von Fehlbildungen und Anlagestörungen der Brust sowie zur kosmetischen Augmentation. Für die Implantation von Silikonprothesen spricht der geringe operative Aufwand, die niedrige Komplikationsrate, die zunehmend besseren Möglichkeiten der Anpassung an die natürliche Form der Brust sowie die bessere biologische Kompatibilität durch die Verwendung moderner Materialien und Oberflächenbeschaffenheiten. Nachdem in den USA in den 60er-Jahren erstmals Silikonprothesen zur kosmetischen Augmentation und zur Brustrekonstruktion implantiert wurden, betrug die Zahl der Implantatträgerinnen im Jahr 1989 in den USA 815 700. Zur Steigerung der Biokompatibilität sind lokale Komplikationen wie die Kapselfibrose und die Leakage von Silikonbestandteilen in den umgebenden Weichteilmantel Gegenstand intensivster Forschung. So wurde die Rate an Kapselkontrakturen durch die Einführung texturierter Oberflächen und durch die Verwendung von doppellumigen Prothesen mit einem silikongelgefüllten Kern und einer NaCl-gefüllten Mantelzone erheblich gesenkt.

Infolge eines Beschlusses der FDA vom 16. 4. 1992 wurden in den USA Silikonprothesen lediglich zur Brustrekonstruktion und nicht mehr zur Brustvergrößerung zugelassen. Begründet wurde dies mit den nicht ausreichend untersuchten Auswirkungen der Silikonimplantate auf das umgebende Gewebe, deren Kanzerogenität sowie deren pathogenetische Bedeutung für autoimmunologische Erkrankungen und Krankheiten des rheumatischen Formenkreises. Seit dieser Entscheidung gab es eine große Anzahl von Untersuchungen zur Erforschung der Biokompatibilität von Silikonprothesen, in deren Ergebnis Silikon als auch Polyurethan als die am besten erforschten Biomaterialien angesehen werden können. Trotz umfangreicher wissenschaftlicher Bestätigung der Sicherheit von Brustimplantaten stehen sowohl das Implantatmaterial als auch die Implantate selbst im Kreuzfeuer der öffentlichen Kritik.
