Fallbericht zur Differenzialdiagnose der gastroösophagealen Refluxkrankheit: die eosinophile Ösophagitis

Zusammenfassung

Ein 22-jähriger Patient klagte seit 10 Jahren über Dysphagie und rezidivierende Bolusimpaktion in der Speiseröhre. Wiederholt war endoskopisch ein Bolus in den Magen vorgeschoben worden. Nach Diagnose einer Refluxösophagitis IV° und Einleitung einer säurehemmenden Medikation, unter der es zu keiner Beschwerdebesserung kam, wurde der Patient zur Klärung der Indikation einer Fundoplicatio bei uns vorgestellt. pH-metrisch konnte ein gesteigerter gastroösophagealer Reflux nachgewiesen werden. Gastroskopisch zeigten sich rötliche Straßen im Ösophagus, die makroskopisch keinen Erosionen entsprachen. Manometrie und Ösophagusbreischluck erbrachten Normalbefunde. Histologisch konnte die in Verbindung mit Anamnese, Klinik und Endoskopiebefund typische Diagnose einer eosinophilen Ösophagitis gestellt werden. Unter Kortisontherapie kam es zur kompletten Remission der Symptomatik. Der Fallbericht zeigt, dass trotz pH-metrisch nachgewiesenem pathologischen Reflux bei medikamentenrefraktären Beschwerden und wiederholter Bolusimpaktion auch an die seltene Differenzialdiagnose der eosinophilen Ösophagitis gedacht werden muss.

Abstract

We report on a 22-year-old man with dysphagia and repeated bolus impaction in the esophagus for 10 years. Bolus impactions were frequently mobilised using an endoscope. At endoscopy, esophagitis IV° was described. After treatment with omeprazol there was no improvement. The patient was submitted to our hospital for fundoplicatio. pH-metry demonstrated an increased reflux. At endoscopy of the esophagus, we found red stripes which did not show the typical appearance of erosions. Manometry and X-ray films of the esophagus did not reveal any pathological findings. In combination with anamnesis, symptoms, and endoscopy, the diagnosis of eosinophilic esophagitis was documented by histology. After administration of oral corticosteroids a rapid improvement of the clinical symptoms was observed. The diagnosis of eosinophilic esophagitis should be kept in mind in patients with chronic symptoms of gastroesophageal reflux persisting despite medical therapy, pathological pH-metry and repeated bolus impactions.
