30 - 50 % der Patienten mit typischen Symptomen der Herzinsuffizienz weisen eine normale oder nur geringgradig reduzierte linksventrikuläre Ejektionsfraktion auf [36] [43]. Überwiegend ist bei ihnen von einer diastolischen Funktionsstörung als Ursache der Herzinsuffizienz auszugehen [32]. Die meisten Patienten mit normaler linksventrikulärer Ejektionsfraktion, bei denen sich eine Herzinsuffizienz entwickelt, sind ältere Frauen [19]. Heute ist die chronische Herzinsuffizienz bereits die häufigste Ursache einer Hospitalisierung bei über 65-Jährigen [25]. In Anbetracht der zu erwartenden Altersentwicklung und der stagnierenden Inzidenz der chronischen Herzinsuffizienz bei gleichbleibend schlechter Prognose im höheren Alter [34] dürfte die diastolischen Herzinsuffizienz zukünftig noch erheblich an Bedeutung gewinnen.
