Die Radiosynoviorthese im Spiegel der Evidenz-basierten Medizin (EbM)

Zusammenfassung

Die vorliegende Arbeit fasst die verfügbare Literatur zur Wirksamkeit der Radiosynoviorthese nach Gesichtspunkten der Evidenz-basierten Medizin zusammen. Gerade in Zeiten zunehmender Verknappung finanzieller Mittel im Gesundheitssystem muss sich jede Therapie dem Vergleich mit anderen Methoden stellen. Hier erscheint vielen die Evidenz-basierte Medizin, d. h. die Gewichtung publizierter klinischer Studien nach bestimmten Kriterien wie der Randomisierung, Verblindung oder Kontrolle durch Vergleich mit einem Plazebo gewissermaßen als „Stein der Weisen”. Für die Radiosynoviorthese wird die Existenz qualitativ hochwertiger Studien häufig verneint - eine Aussage, die jedoch nach genauem Studium der verfügbaren Literatur nicht zu halten ist. Dem in der RSO tätigen Nuklearmediziner sollen durch die Auflistung und kurze inhaltliche Beschreibung einer Reihe von Studien der Evidenzgrade I b und II a wichtige Argumente an die Hand gegeben werden, die die eigene klinische Erfahrung mit dieser nuklearmedizinischen Therapie wissenschaftlich stützen und als Rüstzeug in der Diskussion mit Kollegen anderer Fachdisziplinen dienen sollen. Eine kritische Betrachtung der Evidenz-basierten Medizin als alleinige Grundlage des therapeutischen Handelns rundet die Arbeit ab.

Abstract

This paper summarizes the literature on clinical efficacy of radiosynoviorthesis with respect to the criteria of evidence-based medicine (EbM). Each therapy has to face up to the success and the costs of other treatment modalities, especially in times of scarce financial resources of our health care system. For many physicians and scientists, evidence-based medicine, which means weighting clinical trials by several quality factors like randomization, blinding or comparison with a placebo seems to be the “philosophers'stone” to some extent. Concerning radiosynoviorthesis, the existence of clinical studies with a high quality according to the EbM is sometimes negated. However, this statement cannot be abided after extensive revision of the literature. The nuclear medicine physician practising radiosynoviorthesis should be provided with arguments from the literature supporting his personal experience for successful discussions with referring collegues. This is achieved by listing the respective papers especially of evidence classes I b and II a with a short summary of each of them. Some critical remarks on evidence-based medicine if used as the only basis of therapeutic practise complete this paper.
