Evidenz für den Einsatz des Ultraschalls in der Physiotherapie

Zusammenfassung

Hintergrund: Für positive Effekte der Ultraschalltherapie hat die Grundlagenforschung deutliche Beweise hervorgebracht. Eine Metaanalyse älteren Datums zeigt jedoch, dass die klinische Evidenz zur Anwendung der Ultraschalltherapie unzureichend ist.

Ziel: Die vorliegende Arbeit soll das aktuelle Evidenzniveau für die Indikationsstellungen der Ultraschalltherapie bestimmen.

Methode: Für die Datenbanken Medline und The Cochrane Database of Systematic Reviews wurden Suchschlüssel aufgestellt, mithilfe derer nach 1996 veröffentlichten systematischen Reviews gesucht wurde. Die Beurteilung der Studienqualität erfolgte nach zuvor festgelegten Kriterien.

Ergebnisse: Anhand der 11 aufgenommenen Publikationen liegt für den Einsatz des Ultraschalls in der Frakturbehandlung, bei kalzifizierender Tendinitis im Schulterbereich und für die Behandlung bei Epicondylitis lateralis klinische Evidenz vor, für andere Indikationsstellungen bleibt die Wirksamkeit unklar.

Schlussfolgerung: Für die Mehrzahl empirisch ermittelter Indikationsstellungen sind qualitativ hochwertige, klinische Untersuchungen erforderlich.

Abstract

Background: For positive effects of ultrasound therapy basic research has established clear preclinical proof. However, an older meta-analysis showed that clinical evidence for the application of ultrasound is insufficient.

Objective: This review intends to determine the current level of evidence for the indication of therapeutic ultrasound.

Method: For the electronic databases Medline and The Cochrane Database of Systematic Reviews a search strategy was developed by which systematic reviews published after 1996 were searched for. The quality of the publications was evaluated according to criterions fixed in advance.

Result: On the basis of 11 publications the use of ultrasound in fracture treatment, intervention in calcific tendinitis in the shoulder and lateral epicondylitis clinical evidence is available, for other indications the effectiveness remains uncertain.

Conclusion: Most of the empirically determined indications require high-quality clinical studies.
