Komplikationen der Ohrmuschelanlegeplastik

Zusammenfassung

Das Komplikationsrisiko der Ohrmuschelanlegeplastik wird unterschätzt; es ist in ca. 5 % mit Fühkomplikationen (Hämatom, Infektion, Fadenfisteln und Granulomen, allergischen Reaktionen, Druckulzera, Schmerzsensationen, Asymmetrien im Seitenvergleich) und in über 20 % mit Spätkomplikationen zu rechnen (Rezidive, Telefonohr, übermäßige Kantenbildung, zu starkes Anliegen der Ohrmuschel, Einengung des Gehörgangs, Keloid und „Katastrophenohr”). Deformitäten werden, soweit sie nicht den Anlegeeffekt betreffen, vom Patienten weniger kritisch eingeschätzt als vom Operateur. Ursachen von Komplikationen und Deformitäten sind überwiegend falsche Indikationsstellung und falsche Auswahl des Operationsverfahrens. Die Wahl des Operationsverfahrens muss der individuellen Ohrmorphologie angepasst sein. Für das Auftreten und den Verlauf von Frühkomplikationen sind neben der Operationstaktik auch Verbandstechnik und Kontrolle, gegebenenfalls Frührevision entscheidend. Bei Spätkomplikationen wie Keloid und zu stark anliegender Ohrmuschel haben sich freie Vollhautlappen am besten bewährt. Starke Deformitäten lassen häufig nur noch eine Korrektur mit eingeschränkt zufriedenstellendem Ergebnis zu.
