Was sind Biologicals?

Der Begriff „Biologicals” umfasst eine neuartige Gruppe Therapeutika, deren Molekularstruktur häufig aus Bestandteilen besteht, die natürlicherweise Teile anderer biologischer Moleküle sind oder die gentechnisch hergestellt, natürlich vorkommende Moleküle imitieren. Biologicals werden gezielt gegen definierte Zielstrukturen entwickelt und sind somit echte „Designermoleküle”: Ihre Entwicklung und Produktion wurde erst durch die enormen Fortschritte in der Molekularbiologie und -pharmazie in den letzten Jahren möglich. Biologicals haben vor allem bei der Behandlung von Autoimmunerkrankungen auch bei Kindern neue Behandlungsoptionen eröffnet und damit die erfolgreiche Therapie von Patienten ermöglicht, bei denen die herkömmlichen Basistherapeutika keinen ausreichenden Effekt erzielen [1] [2]. Die Zielstrukturen der heute verfügbaren Biologicals sind zumeist Signal- und Differenzierungsfaktoren des Immunsystems. Sie hemmen selektiv zentrale inflammatorische Signalwege. In naher Zukunft ist mit einer Vielzahl von Neuentwicklungen innerhalb dieser Substanzgruppe zu rechnen, die sicherlich auch Entwicklungen gegen andere Zielstrukturen beinhaltet. Im Folgenden sollen die aktuell in der Pädiatrie verwendeten Biologicals vorgestellt und am Beispiel von Prototypen die Wirkmechanismen, Indikationen und Risiken dieser Substanzgruppe erläutert werden.
