Management und sozialpolitische Aspekte sexuell übertragener Infektionen

Einleitung

Annähernd eine Million Menschen infiziert sich täglich weltweit mit einer sogenannten Sexually transmitted Infection (STI), wobei in diese Zahl auch die Infektion mit dem HI-Virus (Human Immundeficiency Virus) eingeht.

Im Gefolge dieser Infektionen kommt es zu akuten Symptomen ebenso wie zu chronischen Verläufen, aber auch zu schwerwiegenden, mit Verzögerung auftretenden Komplikationen, wie

Abszedierung im kleinen Becken, Infertilität, ektopische Gravidität, Zervixkarzinom und schlussendlich zum Tod von Kindern und Erwachsenen.

Im Übrigen bedeutet die Erkrankung einer Person an einer STI, wie zum Beispiel Syphilis, Chancroid oder Herpes genitalis, einen erheblichen Anstieg des Risikos, HIV zu akquirieren oder zu übertragen, insbesondere bei Frühformen der Infektion mit HIV bzw. entsprechend hoher Viruslast [12]. Diese Konstellation dürfte mindestens 40 % aller HIV-Übertragungen betreffen.

Die Erkrankung an einer STI erhöht das Risiko beträchtlich, sich auch mit HIV zu infizieren.

Im Rahmen dieser Übersicht kann und soll nicht auf die Vielfalt aller STI eingegangen werden. Vielmehr sollen neben dem Repetitorium aktuelle Aspekte aus der Praxis in Deutschland stehen, wie sie sich für den Frauenarzt täglich überraschend ergeben können und wie sie ein zügiges, zielstrebiges Handeln oder eine Weichenstellung hin zur Fachabteilung verlangen.
