Reaktivierung einer Hepatitis C unter Chemotherapie eines Mammakarzinoms

Zusammenfassung

Bei einer 34-jährigen Patientin mit Mammakarzinom wurde eine adjuvante Chemotherapie mit 3 Zyklen FEC und 3 Zyklen Docetaxel durchgeführt. Bei der Patientin war in der Vorgeschichte ein Morbus Hodgkin im Alter von 14 Jahren sowie eine Hepatitis-C-Infektion bekannt. Zu Beginn der Chemotherapie war die Patientin HCV-RNA negativ. Unter der Chemotherapie mit Docetaxel kam es zu einem Widerauftreten von HCV-RNA ohne Anstieg der Leberenzyme. Nach Abschluss der Therapie wurde ohne weitere therapeutische Maßnahmen bei bleibendem Verlauf wieder eine komplette Negativierung gesehen. Im Vergleich zur Hepatitis B liegen nur wenige Daten zur Häufigkeit der Reaktivierung ihrer Bedeutung und keine Therapieempfehlungen vor. Eine medikamentöse Therapie scheint in den meisten Fällen nicht erforderlich.

Abstract

A 34 year old patient with breast cancer was treated with 3 cycles FEC and 3 cycles docetaxel. At the age of 14 she had been trated for M. Hodgkin, at the age fo 29 Hepatitis C was diagnosed and treated. Starting chemotherapy she was negative for HCV-RNA. During treatment with docetaxel HCV-RNA became positive, liver enzymes did not rise. Shortly after end of treatment HCV-RNA became negative again without any specific treatment. Compared to hepatitis B there are only few reports about the incidence of reactivation of hepatits C and treatment. A medical treatment does not seem to be necessary in most cases.
