Zusammenfassung

Ziel der Studie war es, eine Untersuchung zur Inzidenz des Typ-1-Diabetes bei Kindern im Alter von 0–14 Jahren in dem Ost- und Westteil der Stadt im Jahr 1988 sowie in Gesamt-Berlin in den Jahren 1996–2000 durchzuführen. Es wurden im Sinne einer Totalerhebung alle Kinder im Alter von 0–14 Jahren mit Wohnsitz in Berlin in die Untersuchung aufgenommen, die in diesen sechs Untersuchungsjahren in den neun Berliner Kinderkliniken und Kinderpolikliniken wegen Manifestation eines Typ-1-Diabetes behandelt wurden. Als primäre Datenquelle dienten die Aufnahmebücher der Kliniken, als sekundäre die in Berlin niedergelassenen Kinderärzte (1988 und 1996–1999) bzw. die Selbsthilfeorganisation „Bundesweite Fördergemeinschaft junger Diabetiker e. V.” (2000). Die Daten wurden nach Richtlinien der EURODIAB-ACE-Studie, dem WHO-DIAMOND-Projekt und der DERI-Gruppe erhoben und ausgewertet. Die Erfassungsgenauigkeit wurde mit der Capture-Recapture-Methode geprüft. Insgesamt wurden die Daten von 376 Kindern (166 Mädchen, 210 Jungen) mit Manifestation eines Typ-1-Diabetes retrospektiv erhoben. Die Erfassungsgenauigkeit lag bei 97–100 %. Die geschlechts- und altersstandardisierte Inzidenzrate (r / 100 000 / a) stieg von 6,7 (95 %-KI: 4,6–9,4) in 1988 bis auf 13,2 (10,1–17,0) in 2000 an. Erstmals liegen hiermit für Berlin standardisierte Daten zur Inzidenzrate des Typ-1-Diabetes bei Kindern vor. Die Berliner Daten bestätigen die allgemeine Zunahme der Diabetes-Inzidenz in den letzten Jahrzehnten sowohl in Deutschland als auch in anderen europäischen Ländern.

Abstract

In this study, the incidence of type 1 diabetes in children aged 0 to 14 years living in Berlin was estimated in the years 1988 and 1996–2000, respectively. Clinical records from nine children hospitals were the primary data source. As secondary independent data source, a questionnaire was circulated among paediatricians (1988 and 1996–1999) and the patients' association “Bundesweite Fördergemeinschaft junger Diabetiker e. V.” (2000). Case definition and evaluation was done according to the criteria of the EURODIAB ACE study, the WHO-DIAMOND project and the DERI Group. The degree of ascertainment estimated by the capture-recapture method ranged between 97 and 100 %. A total of 376 children (116 girls, 210 boys) with newly diagnosed type 1 diabetes were identified. The gender- and age-standardized annual incidence (per 100 000 children) increased from 6.7 (95 %-CI: 4.6–9.4) in the year 1988 to 13.2 (10.1–17.0) in the year 2000. For the first time, we report standardized data on the incidence of type 1 diabetes in children up to 15 years of age for the Berlin population. These data confirm the increasing incidence of diabetes in children as previously reported for other German regions and European countries.
