Quantifizierung des Outcomes phonochirurgischer Interventionen bei benignen Stimmlippenläsionen

Die mikrolaryngoskopische Abtragung benigner Stimmlippenläsionen führt zu einer Stimmverbesserung. Um zu zeigen, in welchem Ausmaß sich die Stimme bessert, wurden subjektive und objektive Stimmparameter vor und nach Entfernung von Stimmlippenpolypen, Stimmlippenknötchen und Reinke-Ödemen erfasst. Das in der klinischen Routinediagnostik noch nicht allgemein etablierte Stimmumfangsmaß (SUM) hat im Vergleich zu bereits etablierten Parametern einschließlich des Dysphonie-Schweregrad-Indexes (DSI) eine gleichwertige Aussagekraft.

Abstract

The microlaryngoscopic excision of benign vocal fold lesions improves the voice. In order to show to which extent patients eventually benefit, the magnitude of the impact of phono-microsurgery in vocal fold polyps, vocal fold nodules and Reinke's edema on subjective and objective parameters of vocal function was investigated. Particular regard was given to the vocal extent measure (VEM), a parameter not yet commonly established in phoniatric diagnostics, whose performance was equal to that of established vocal parameters including the dysphonia severity index (DSI).

Fazit

Die Ergebnisse lassen schlussfolgern, dass die mikrolaryngoskopische Abtragung der untersuchten benignen Stimmlippenläsionen bei korrekter Diagnosestellung, fachgemäßem Vorgehen während der Operation sowie bei regelrechtem postoperativem Verlauf eine sichere, quantifizierbare, subjektiv und objektiv höchst zufriedenstellende Therapie zur Stimmverbesserung darstellt. Der Parameter SUM zeigt signifikante Veränderungen bei phonomikrochirurgischer Behandlung und erweist sich als verständlicher und einfach anzuwendender neuer Parameter. Er weist eine Reihe von Vorteilen gegenüber dem DSI auf und erscheint überaus geeignet, als Ergänzung zu anderen etablierten Stimmparametern die stimmliche Leistungsfähigkeit objektiv zu quantifizieren.
