Zusammenfassung

Ein 72jähriger Patient erwacht morgens mit Thorax- und Nackenschmerzen. Bei Eintreffen des Rettungsdienstes wird er mit gering eingeschränkter Vigilanz (GCS14) im Sessel sitzend vorgefunden. Der Blutdruck liegt bei 185/100 mmHg, die Herzfrequenz bei 40/min und die Atemfrequenz schwankt irregulär zwischen 13 und 18/min im Sinne einer Cushing-Trias. Die sofortige CT-Diagnostik nach Einlieferung in die Notaufnahme ergibt eine ausgeprägte Subarachnoidalblutung. Eine sorgfältige präklinische Einschätzung und Verdachtsdiagnose erlaubt die zielgerichtete Weiterversorgung.

Abstract

A 72-year-old patient wakes up in the morning with chest and neck pain. When the emergency medical service arrives, he is found sitting in an armchair with slightly reduced vigilance (GCS14). The blood pressure is 185/100 mmHg, the heart rate is 40/min and the respiratory rate varies irregularly between 13 and 18/min in the sense of a Cushingʼs Triad. Immediate CT-diagnosis after admission to the emergency department reveals a pronounced subarachnoid hemorrhage. Careful preclinical evaluation and suspected diagnosis allows for a targeted admission.
