Oscar Nogales et al. Cold snare endoscopic mucosal resection versus standard hot technique for large flat nonpedunculated colonic lesions: a randomized controlled trial

The optimal treatment of large flat nonpedunculated colonic lesions (LFNPCLs) ≥ 20 mm is still debated, with multiple recent studies investigating the utility of cold snare endoscopic mucosal resection (CS-EMR). In this multicenter randomized controlled trial, CS-EMR was compared with standard hot snare EMR for 229 LFNPCLs, mostly adenomas (74.6 %). Recurrence at the first surveillance colonoscopy at 6 months was significantly greater for CS-EMR (33.0%) than for standard EMR (16.2%), with no difference in the rate of adverse events.
