Summary

The laparoscopic photographs of 38 cases of primary biliary cirrhosis with high titres of mitochondrial antibodies were reviewed. The quality of laparoscopic photography is now so high that it proved possible to use these photographs as the basis of a system of laparoscopic staging. By analogy with the system of histological classification, the disease was divided into four stages. Laparoscopy provides an alternative approach to the diagnosis of primary biliary cirrhosis, which should be based on clinical, biochemical, immunological and morphological data. However, by laparoscopy alone it is improssible to separate pure primary biliary cirrhosis from its mixed forms with chronic active hepatitis.

Zusammenfassung

Die laparoskopischen Aufnahmen von 36 Patienten mit primär biliärer Zirrhose und hohen Titern an antimitochondrialen Antikörpern wurden analysiert. Die Qualität dieser Aufnahmen ist heute so hoch, daß sie als Grundlage für eine laparoskopische Einteilung in verschiedene Stadien herangezogen werden können. In Analogie zu dem histologischen Klassifikationssystem erfolgte eine Einteilung in 4 Stadien. Die Laparoskopie stellt eine Alternativmethode bei der Diagnostik der primär biliären Zirrhose dar, die noch durch klinische, biochemische und immunologische sowie morphologische Daten ergänzt werden sollte. Allein mit der Laparoskopie ist es jedoch nicht möglich, die reine biliäre Zirrhose von ihren Mischformen mit einer aktiv chronischen Hepatitis zu differenzieren.
