Zum Problem der Verminderung des Blutverlustes nach Operationen mit Hilfe des extrakorporalen Kreislaufs

Zusammenfassung
Anhand der Befunde von 120 Patienten, die mit Hilfe des extrakorporalen Kreislaufs operiert wurden, wird auf die Möglichkeiten zur Verminderung von postoperativen Blutverlusten hingewiesen. Durch Änderungen der Perfusionstechnik, durch Anwendung einer exakten Heparin-Protamin-Titrationsmethode, durch Erhöhung der Heparindosis, durch regelmäßige Fibrinogengaben sowie durch die prophylaktische und therapeutische Anwendung von intravenös applizierbarer ∈-Amino-capronsäure gelang es bei 66 Patienten die durchschnittliche Nachblutungsmenge auf 11,1 ml/kg Körpergewicht gegenüber 28,7 ml/kg Körpergewicht bei 54 vorher operierten Patienten zu senken. Während bei den verglichenen Patientengruppen bei zyanotischen Vitien der durchschnittliche post-operative Blutverlust nur wenig verringert werden konnte, gelang es bei den azyanotischen Vitien den Blutverlust auf 7,6 ml/kg Körpergewicht zu senken, d. h. auf Werte, die auch für herzchirurgische Eingriffe ohne Herz-Lungen-Maschine durchaus normal sind.
Summary
The possibilities of reducing postoperative blood loss after operations under conditions of extracorporeal circulation are outlined, based on the findings in 120 patients. By changing the perfusion technique, application of an exact heparin-protamin titration method, regular administration of fibrinogen, increase of the heparin dose and by prophylactic and therapeutic administration of Eaminocaproic acid applicable intravenously we succeeded in reducing the postoperative blood loss in a series of 66 patients to 11.1. ml/kg body weight as compared to 28.7 ml/kg in a series of 54 patients operated upon previously. In cyanotic lesions the average postoperative blood loss decreased only slightly, but in acyanotic lesions an average of 7.6 ml/kg bodyweight could be achieved. This is a value normal in cardiovascular surgery without cardiopulmonary by-pass.
Résumé
Nous indiquons les possibilités de diminuer les pertes postopératoires de sang chez des patients operes avec l'aide d'une circulation extracorporelle. II s'agit d'une étude sur 120 patients. Nous parvenons ä faire descendre en moyenne la perte de sang ä 11,1 ml./Kg. chez 66 patients. Nous obtenions 28,7 ml./Kg. chez les 54 operes avant les ameliorations que nous allons citer: une amelioration dans la technique de perfusion, une utilisation d'une méthode exacte de titration concernant l'héparine protamine, une elevation de la dose d'héparine, un apport regulier de fibrinogéne, une utilisation prophilactique et thérapeutique d'une intraveineuse d'acide e-aminocapron. Si en comparant les deux groupes de patients, la perte de sang peut étre seulement légérement diminuée dans le vitius cyanotique on parvient dans le vitius non cyanotique ä baisser la perte de sang ä 7,6 ml/Kg. Cette valeur est pratiquement normale puisque on l'obtient dans les interventions cardiaques sans coeur-poumon artificiel.
