Multiple Hirnnervenverletzungen und Dissektion der A.carotis interna bds. durch Pfählungsverletzung der Ohren

Zusammenfassung

Es wird der Fall eines 31-jährigen Patienten mit schizoaffektiver Psychose und akustischen Halluzinationen präsentiert, der sich durch selbst eingebrachte Schraubenzieher in beide Gehörgänge eine penetrierende, offene Schädel-Hirn-Verletzung mit hochgradigen bis kompletten Läsionen der Hirnnerven V, VI, VII und VIII beidseits und mit asymptomatischen Dissektionen der A. carotis interna beidseits zufügte. Der Fall beschreibt den hohen Leidensdruck schizoaffektiver Patienten sowie das komplexe Ausfallmuster traumatischer Hirnnervenläsionen im Kontext zur aktuellen Literatur.

Abstract

We present the case of a 31-year-old man with a schizoaffective disorder and a penetrating head injury by 2 self-inserted skrewdrivers in both external auditory canals with a palsy of the cranial nerves V, VI, VII and VIII on both sides and an asymptomatic dissection of both carotid arteries. This case demonstrates the high psychological strain of patients with schizoaffective disorders and the complex neurological deficits of traumatic cranial neuropathies in the context of the current literature.
