Zusammenfassung

Die akute respiratorische Insuffizienz auf dem Boden obstruktiver Ventilationsstörungen ist ein häufig vorkommendes Krankheitsbild im Rettungsdienst. In den meisten Fällen sind dabei der akute Asthmaanfall und die akut exazerbierte chronisch obstruktive Lungenerkrankung (chronic obstructive pulmonary disease, COPD) ursächlich. Eine genaue Differenzierung von Asthma bronchiale und COPD kann präklinisch bei eingeschränkten diagnostischen Möglichkeiten schwierig sein, sodass die Verdachts-diagnose häufig auf Eigen- oder Fremdanamnese basiert. In dieser Arbeit soll insbesondere auf die präklinischen Basismaßnahmen, die pharmakologischen Therapie-optionen und auf die apparativen Verfahren einer Atmungsunterstützung (nicht invasiv vs. invasiv) eingegangen werden.

Abstract

Acute respiratory insufficiency due to obstructive pulmonary diseases is a common problem presenting to the emergency medical service. Most frequent causes are acute asthma attacks or acute exacerbations of chronic obstructive pulmonary disease (COPD). The preclinical differentiation of both diseases may be difficult, so that the diagnosis is often made by precise anamnesis. This article reviews the prehospital management of asthma and COPD, including pharmacological options and techniques of mechanical ventilatory support (non-invasive vs. invasive).

  • Die häufigsten Ursachen von obstruktiven Atemwegserkrankungen sind der akute Asthmaanfall und die Exazerbation einer COPD.

  • Die präklinische Unterscheidung zwischen einem akuten Asthmaanfall und einer exazerbierten COPD ist häufig nur anhand anamnestischer Daten möglich.

  • Während beim Asthma bronchiale nur eine geringe Gefahr der Hyperkapnie durch zu hohe O2-Flussraten besteht [7], ist bei der COPD eine kontrollierte O2-Therapie bei einer pulsoxymetrischen O2-Sättigung von < 90 % indiziert.

  • Die medikamentöse Initialtherapie sollte aus einer inhalativen Applikation von schnellwirksamen ß2-Sympathomimetika und der systemischen Kortikosteroidgabe bestehen.

  • Theophyllin sollte bei vorbestehender Theophyllinmedikation präklinisch nicht angewendet werden.

  • Der präklinische Einsatz der nicht invasiven Beatmung sollte bei fehlenden Kontraindikationen bei Patienten mit exazerbierter COPD erwogen werden.

  • Zur präklinischen Anwendung der nicht invasiven Beatmung beim akuten Asthmaanfall kann derzeit keine generelle Empfehlung ausgesprochen werden.

  • Eine notwendige Intubation darf durch einen NIV-Versuch nicht verzögert oder verhindert werden.

  • Induktionshypnotika der Wahl bei Patienten mit obstruktiven Ventilationsstörungen sind Propofol oder Ketamin.

  • Bei der invasiven Beatmung von Patienten mit obstruktiven Atemwegserkrankungen sollten der Spitzendruck limitiert sowie niedrige Atemfrequenzen und eine verlängerte Exspirationszeit gewählt werden.
