Obwohl das akute Abdomen mit knapp 10 % eine der häufigsten Notfallsituationen darstellt, fehlen deutsche wie auch europäische Leitlinien zum diagnostischen Vorgehen. Das Prozedere wird oft mehr von tradierten Gewohnheiten und lokaler Expertise bestimmt als von den wenigen validen Studien. Alte Zöpfe wie die meist überflüssige Abdomen-Leeraufnahme lassen sich so nur schwer ausrotten. Der folgende Artikel beleuchtet anhand von 3 häufigen Verdachtsdiagnosen (Appendizitis, Divertikulitis und Ileus) beispielhaft den Stellenwert der Notfallsonographie, insbesondere im Vergleich zu radiologischen Konkurrenzmethoden. Was ist möglich? Was ist evidenzbasiert? Und was ist klinische Realität?

Although acute abdominal pain with nearly 10 % represents one of the most common emergency situations, no German and European guidelines exist for diagnostic procedure in these cases. Clinical practice is more determined by local expertise and traditional habits than by the few reliable studies. Ancient habits – like the in most cases unnecessary plain film - may be difficult to eradicate. The following article illustrates the importance of emergency ultrasound in 3 exemplary situations: appendicitis, diverticulitis and bowel obstruction. Particular attention is given to the comparison with the radiological methods. What is possible? What is evidence-based? And how does it look in real life?
