Zusammenfassung

Die transorale minimal-invasive Chirurgie führt zu einer erheblichen Minderung des Operationstraumas im Vergleich zu transzervikalen offenen Zugängen sowie zu einem Paradigmenwechsel in der operativen Therapie von Kopf-Hals-Tumoren. In selektierten Fällen erlaubt sie zudem, die Morbidität der Patienten ohne Verschlechterung der onkologischen Ergebnisse zu reduzieren. Dadurch erfahren die transoralen Operationstechniken eine zunehmende Akzeptanz und eine Erweiterung der Indikationsstellung. Infolgedessen entwickeln sich neue chirurgische Verfahren mit Verbesserung der dazu erforderlichen Instrumente.

Im vorliegenden Artikel werden die transoralen Therapiestrategien in Abhängigkeit von ihren anatomischen Lokalisationen und deren Indikationen beschrieben sowie Angaben aus der Literatur hinsichtlich Entwicklung und Ergebnisse diskutiert.

Abstract

Transoral Treatment Strategies in Head and Neck Tumors

The introduction of transoral endoscopic surgery resulted in a significant reduction of surgical trauma and a paradigm change from transcervical to transoral surgical treatment in head and neck tumors. In selected cases minimally invasive transoral resections allow a reduction of morbidity without deteriorating oncological results. Thus, transoral surgical techniques gained more acceptance, led to an extension of the indications and technologies as well as an improvement of surgical instruments.

Depending on the anatomical localization transoral treatment strategies and their indications are described and relevant literature is discussed according to new developments and results.
