Die Diagnostik des Mammakarzinoms basiert neben der klinischen Untersuchung auf bildgebenden Verfahren. Die wichtigste Untersuchungsmethode stellt hierbei die Mammografie dar, welche durch die Mammasonografie und ggf. die KM-MRT ergänzt wird. Die Mammografie spielt dabei vor allem eine wichtige Rolle im Rahmen des Screenings asymptomatischer Frauen. Die histologische Diagnosesicherung erfolgt durch die sonografische Stanzbiopsie, stereotaktische Vakuumbiopsie oder selten auch durch die offene Biopsie. Im Falle eines Karzinoms bestehen dann operativ grundsätzlich 2 Möglichkeiten: eine Mastektomie oder ein brusterhaltendes Vorgehen, jeweils in Kombination mit der Sentinellymphknoten-Biopsie und ggf. Axilladissektion. Letztere hat große diagnostische Bedeutung, da der Nodalstatus als wichtigster Prognoseparameter in hohem Maße Einfluss auf die adjuvante Systemtherapie hat.

Besides clinical examination the diagnostic procedure of breast cancer is based on imaging techniques with a focus on mammography followed by breast ultrasound, and in addition by phase-contrast MRI. The mammography is particularly important concerning the breast cancer screening of asymptomatic women. Histological confirmation of diagnosis is performed using ultrasound-guided core needle biopsy, stereotactic biopsy or sometimes as open biopsy. In case of cancer, there are basically two different ways of doing surgery: mastectomy or breast conserving surgery, each time in combination with sentinel lymphnode-biopsy and where appropriate with axillary lymphnode dissection. The latter has substantial diagnostic implications, since the nodal status as most important prognostic factor influences the adjuvant systemic therapy significantly.
